However, the presentation of this metastatic cancer in the form of diffuse gastric polyposis is even more uncommon. In this case report, we present a unique and intriguing case of metastatic gastric mucosal melanoma with diffuse gastric polyposis.

Introduction

Metastatic gastric mucosal melanoma is a condition characterized by the spread of melanoma cells to the stomach. It typically occurs as a result of metastasis from cutaneous or ocular melanoma. However, the presentation of this metastasis as diffuse gastric polyposis is exceptionally rare, with only a handful of cases reported in medical literature.

Case Report

The patient, a 54-year-old male, presented with complaints of intermittent abdominal pain and weight loss. Initial investigation revealed multiple polyps throughout the stomach, leading to a suspicion of familial adenomatous polyposis (FAP). However, biopsy results unexpectedly revealed the presence of metastatic gastric mucosal melanoma.

Discussion

Diffuse gastric polyposis is a condition characterized by the presence of numerous polyps throughout the stomach. It is most commonly associated with FAP, an inherited condition caused by mutations in the APC gene. However, in this case, the presence of metastatic gastric mucosal melanoma in the form of diffuse gastric polyposis posed a diagnostic challenge. It required careful consideration of other possibilities and extensive histopathological examination to confirm the diagnosis.

The rarity of this presentation highlights the importance of thorough investigation and consideration of differential diagnoses in similar cases. In this instance, the patient’s medical history, including previous melanoma diagnosis, played a crucial role in eventually identifying the metastatic nature of the polyps.

Treatment and Prognosis

Given the advanced stage of metastatic gastric mucosal melanoma, treatment options were limited. The patient underwent a multidisciplinary approach, including surgical resection of the polyps and systemic therapy targeting melanoma cells. However, the prognosis remained poor due to the aggressive nature of metastatic melanoma.

Conclusion

The uncommon presentation of metastatic gastric mucosal melanoma as diffuse gastric polyposis emphasizes the importance of considering rare possibilities in clinical practice.
